    You have three display types to choose from:

    Issue Requirements
    You must submit:

      • A photocopy of your DD-214 and DA 1307; or DD-214 and other military issued documentation showing the following requirement established by the Government of Vietnam (Cessation of Hostilities Military Award Group, 1974):

        • Have performed at least one training or combat jump in Vietnam between 1945 and 1975

    Your photocopy must be mailed or faxed to the address below.
    You may NOT email a digitized copy.
